Taking Action: SDOH Programs Innovate in a New Era

Tackling social determinants of health (SDOH) has been a top-line issue for healthcare leaders, especially during the pandemic, which exposed more disparities across vulnerable populations, including increased social isolation and lack of food and adequate technology, to name just a few. As patients face ongoing social risks, needs, and other repercussions from the pandemic, healthcare organizations are mobilizing in new ways to connect them to healthy food, safe housing, transportation, medication, technology, and more.
