AGENDA OVERVIEW
Best Practices for Launching an Acute Care at Home Program
Presented by: HealthLeaders
10:00 - 11:05 AM EST
About the Session:
Launching an acute care at-home program is tricky. Health system leaders first need to identify the right population and develop screening protocols, then gather the resources—from technology to workflow adjustments to home care partnerships--needed for facilitating care in the home.
This panel will explore how to get a new program up and running, including coordinating remote monitoring and in-person care and delegating care team responsibilities. We'll also address how to meet CMS requirements and qualify for waivers.
Panelists:
Eve Cunningham, MD MBA, Group Vice President and Chief of Virtual Care and Digital Health, Providence
Penni Kyte, Digital Care Strategy Officer, Ballad Health
Jason Little, Hospital at Home Director, Roper St. Francis Healthcare
Eric Wicklund (Moderator), Senior Innovation & Technology Editor, HealthLeaders

Balancing Touches With Technology in an Acute Care at Home Model
Presented by: HealthLeaders
11:10 - 12:10 PM EST
About the Session:
Acute care at home programs require a balance of technology and in-person visits to address a patient's needs. Health system leaders need to design workflows that identify when virtual and in-person services are needed, how remote monitoring data is collected, and when that data requires an escalation in care.
This panel will discuss how patients are monitored remotely by care teams, how to adjust to escalating care needs or concerns, and how to integrate in-person care.
Panelists:
Lauren Hopkins, Assistant Vice President of Virtual Care & Community Engagement, Augusta University Health
Eve Dorfman, Vice President, NYU Langone Health
Eric Wicklund (Moderator), Senior Innovation & Technology Editor, HealthLeaders

Modifying the Acute Care at Home Concept for Sustainability and Long-Term Success
Presented by: HealthLeaders
12:15 - 1:15 PM EST
About the Session:
While many Hospital at Home programs are built on the CMS model, some health systems are creating their own programs or fine-tuning the platform to address long-term sustainability. This includes developing a strategy beyond the CMS waivers that support these programs, understanding what patients should be cared for in the hospital and what patients can be treated at home, and looking for new populations or treatments that can be delivered in a home setting.
This panel will explore how some leading health systems are looking beyond the CMS model to create their own programs and what they're doing to prepare for the expected end of the CMS waivers.
Panelists:
Monique Reese, SVP of home & community care, Highmark Health
