About the Session:
This panel session will focus on care coordination challenges for ambulatory care settings. Communication is a key challenge, including making primary care practices aware when a patient has an emergency room visit, communication between primary care providers and specialists, and communication during transitions of care.
Medication reconciliation is another care coordination challenge to make sure patients are not taking contraindicated drugs. Finally, panelists will discuss care coordination challenges related to social determinants of health such as making resources available for patients and referring patients to community-based organizations.
