About the Session:
This panel session will focus on care coordination for high-risk and rising-risk patients, two patient populations with complex care needs. Although healthcare providers are usually aware of their high-risk patients, they also need to identify their rising-risk patients, who often will need multiple doctors and medications but don’t yet.
Once the population of rising-risk patients has been identified, healthcare providers can activate workflows to prepare to collaborate with multiple providers, enroll patients in multiple services, link patients to resources, provide timely follow-up appointments, and ensure there’s insurance coverage for services. Care coordination goals for high-risk and rising-risk patients include avoiding care delays and missed opportunities for care.

Care Coordination in the Outpatient Setting

This panel session will focus on care coordination challenges and solutions for primary care practices and other outpatient settings. Communication is a key challenge, including making primary care practices aware when a patient has an emergency room visit, communication with specialists, and communication during transitions of care such as from acute care to post-acute care. Medication reconciliation is another care coordination challenge to make sure patients are not taking contraindicated drugs. Finally, panelists will discuss care coordination challenges related to social determinants of health such as making resources available for patients and referring patients to community-based organizations.
